Medical needs and flexible offices

I can't see this anywhere so her is a genuine query.

Has anubody encountered and solved the issue of staff in flexible environments whose clinical needs restrict their workplace flexibility, for example:

Special orthopaedic chairs for a person with back trouble (clinically certified)

Special settings for VDUs for a person with visibility issues (likewise certified)

Both queries are, I believe, genuine: i.e. not individuals hiding behind problems. Both arise in a budget constrained environment so solutions such as orthopeadic chairs for all, or techy tracking of log ons and light etc, are not feasible.

I am really looking for people who have solved it! Help
